Welcome to the provider portal. In order for us to help you look up benefit information, you will need to know the union that the member/client is affiliated with or you will need to know the prefix of letters on the member/client’s medical insurance card. If you know the name of the member’s union affiliation, select it from the drop down list of union names on the left and click submit in order to be directed to each particular union’s benefit explanation page. If you do not know the union’s name, you can try to enter the prefix (usually three letters) in the search box entitled ‘identifier’. When you hit submit, you will also be directed to the union’s specific benefit explanation page, which will have the name of the union listed at the top so you will know it for future reference.
