Initial evaluation, monitoring and follow-up evaluation with required reporting, record keeping, documenting:

*Ongoing monitoring following return to work (if needed):
$100.00 per session

Payment Schedule for Services

If fees are paid by the employer, an invoice will accompany the initial report.

If fees are paid by the client, full payment is due at the time of the first visit.

Insurance billing forms will be completed for those who wish to seek insurance reimbursement, although most insurance providers do not cover a SAP evaluation.

Additional Services

Supervisors’ reasonable suspicion training:
$200.00 per hour

Education for employees:
$150.00 per half-hour
